Referral Process

 

ALLOCATION OF CROSSFIRE CARE PACKS

Majority of Crossfire CARE PACKS are kindly packed and provided by CityHope CARE and these packs are allocated on a referral basis only—we do not take requests from the general public, so please do not pass our phone number out to clients. On occasions, Crossfire Social Support Program staff will make on the spot needs assessment, if clients attend the Crossfire Support Services stores seeking help.  Where necessary, preference will given to families who have not received multiple CARE PACKS during the previous few months; this ensures we can reach as many in need as possible.  Crossfire also offers clients access to our store without cost on occasions where food hampers supply are low.

REFERRING YOUR CLIENTS

Submit the contact form using the online submission below, then our staff will send you a referral form to complete and return to our office. Crossfire Social Support Program staff will make it our business to reply to your request the same day to organise collection.  If you require CARE PACKS after hours outside our office hours Monday to Friday 9am to 3pm, after submitting your form you may contact our office on 07 3088 9665 to check availability and arrange a suitable time for collection.

COLLECTION OF HAMPERS

Preferably, a representative of your agency will collect the hamper, however if this is not feasible, you may arrange with Family Worship Centre Ipswich church leaders for the client to collect the hamper directly. Please ensure the referral form has been submitted and that your agency calls Family Worship Centre Ipswich Church to arrange collection.

IN THE EVENT CLIENTS DETAILS NEED TO BE CONFIDENTIAL

We request that you provide only their surname, suburb, and the number of adults and children in the family for recording purposes. This data helps us obtain funding and accurately report our year-to-year support through to ensure we can replenish these CARE PACKS from CityHope CARE. It also helps identify the occasional case where people may be accessing CARE PACKS through multiple agencies.
